Think of IOP like a “part time” program. It provides you with a half-day level of care. And its structure may allow you to continue other important parts of your life – like work, school, or whatever else you might have on your plate.

The IOP schedule allows you to get treatment several days a week – while keeping up with your responsibilities, and living at home. You come in for three hours, do the work with your group and your therapist, and then the rest of the day is yours.

And if you need more than a half-day of support, there’s an option called partial hospitalization (PHP) that provides treatment for 6 hours a day.

What You Do During those 3 Hours of IOP Treatment

Most of your time each day is spent in group therapy sessions, which is a fundamental part of the IOP model.

This is where you’ll share what you’re going through with a group of people who are all working through similar challenges. You’ll learn new tools for handling the challenges you’re facing, and practice how to use those coping skills in real world situations.

On top of group therapy, you’ll also have a:

  • Weekly 1-on-1 session with your therapist
  • Weekly family session (if appropriate)
  • Check-in with a psychiatrist (around once a month) to adjust medications, if needed
  • Short weekly medical follow-up

So each day you spend most of your time in group sessions, with 1:1 and family sessions spread out across the week.

How Many Days a Week Is IOP?

If you’re fairly stable and able to mostly handle the stresses of day to day life, 3 days a week tends to be a good fit. It provides you with some support while you make progress and eventually taper off towards the end of the program.

4-5 days is common when your symptoms are more severe, or when you’ve just stepped down from a hospital stay or a partial hospitalization program and still want a lot of structure for a period of time while you continue to stabilize and ease back into day to day life.

How does the number of days get decided?

Most programs typically start with an assessment to determine how many days of IOP are necessary to support your current situation. The team looks at what you’re currently struggling with and the severity of your symptoms, and they build the schedule from there.

And then as you work through the program, the schedule can be adjusted based on how much support you need.

For example, if three days turns out to not be enough, or if five days feels like too much once you’ve made a certain amount of progress, the schedule gets adjusted. It’s matched to your needs, and getting this right is something that every good program takes seriously.

Can You Work or Go to School During IOP?

Usually, yes. Keeping your job, your classes – or any other obligations you might have – is a major benefit of the half-day IOP program.

The “outpatient” part means your life doesn’t get put on hold while you get treatment.

Three hours a day leaves the rest of the day open – and many programs run morning or evening tracks so you can fit treatment around a shift or a class. Some programs also run virtually, which cuts the commute and makes the hours easier to slot into a workday. Every program has different options, so we’d suggest giving them a call and confirming what the options are.

Even so, it’s still a big commitment. Some people work full-time the whole way through. Others arrange a lighter schedule while they’re in the program. What works depends on your situation and what you and your team decide together.

How Long Does an IOP Program Last?

The whole program usually runs around 8-12 weeks. Most stays average around 90 days. But that’s not set in stone, and how long you stay depends on your progress. Some folks may be in the program for longer than 90 days, and some may be shorter.

It depends on the severity of your symptoms and the progress you make.

Like we said above, the schedule does not always stay the same for the entire program. The total number of weeks the program runs for, the number of days a week, and the number of hours a day may flex depending on how things are going.

Most IOP programs taper off over time. As you stabilize, the program drops you to fewer days and lighter weeks, so by the end you’re attending the program for less time and handling more on your own. You slowly ease out instead of stopping all at once.
